How to start the process of admission from home?
If you have questions on how to obtain a PRI and Screen, insurance approval or financial questions, please contact our Admissions Department at 585.223-3633. Each applicant is carefully assessed to determine the specific medical, physical, nursing and psychosocial needs. If receiving Home Health Care through a licensed agency, the home care agency can complete the PRI and screen, and obtain the other information from the physician in the community. Again please reach out to us, we will help guide you through the process.
If you or your loved one is in a hospital.
Please let the social worker know you are interested in Crest Manor Living & Rehabilitation Center. The social worker will obtain all of the information needed for us to guide you and your family through the process, or pick up the phone and call us 585-223-3633!
Does Crest Manor utilize agency nursing?
At Crest manor we do not utilize agency nursing our goal is to provide a family like environment where you or your loved one receives care from a consistent staff member of our team.
